The chording, which is the majority of the song, is

Am ==> A, C, E

F ==> A, C, F

Em ==> G, B, E

G ==> G, B, D

As far as the individual notes go, the part that comes in occasionally on the verse is

E----(hold)----F E F E G----(hold)----F E F E C----D----

And the part for the chorus is

C D--C E--C D--C E--C D--C E--C D--C E--C D--C E C D----(hold)---- A G

the first time and

C D--C E--C D--C G--C D--C A--C D--C G--C D--C E C D----(hold)---- A G

the other two times.

(The two dashes just mean shorter holds.)
