I need help in figuring out the names / locations for a few of the MB introductions (especially those highlighted in yellow).

All of you would-be editors please go HERE to view what I have compiled thus far and help me by checking the information against episode videos.  View full size if you have trouble reading the names and locations on the reduced size image . . . believe me, it's plenty big enough to deserve being called "poster size".  THANKS!
